The hosting parent/s of an Acro Kids party must fill in the agreement and indemnity form below for each child participating at least 24 hours prior to the party date.

As the host of the party being held at Acro Kids, I acknowledge and agree to the following conditions on behalf of all guests I have invited to attend the venue, including any children under their care:

I confirm that I will ensure each attending child has parental or guardian consent to participate in all physical activities organised by Acro Kids during the event.

I acknowledge that neither Acro Kids nor its staff will be held liable for any injury or damage sustained by any child or guest under my invitation while participating in the Acro Kids program.

I understand that Acro Kids staff will take all reasonable precautions to ensure the safety of all participants.

In the event of an emergency, I authorise the staff of Acro Kids to act in their best judgement. If a parent or guardian cannot be contacted, I give permission for First Aid to be administered or for a child to be treated by a medical professional at a suitable facility.

I agree that no food or drinks are to be consumed during physical activity. I accept full responsibility for all food and drink bought to the venue and its contents, including ensuring that it does not contain nut products. I acknowledge that Acro Kids is an ALCOHOL FREE venue and no alcohol may be brought into the venue. I acknowledge that Acro Kids accepts no responsibility for any illness or injury resulting from the consumption of any food or drink brought to the venue. I understand it is my responsibility to ensure all food is safely stored and handled.

I acknowledge that I am hosting a party at Acro Kids and am signing this form on behalf of all invited guests, including the parents/guardians of the participating children. I agree to indemnify and hold Acro Kids along with its employees, directors and agents, against all claims arising out of or in connection with participation in these recreational activities.
